Juniper (Juniper Communis) and Heartburn

Juniper berries have traditionally been used in various cultures for their potential medicinal properties, including for digestive problems such as heartburn. The European Medicines Agency (EMA) lists the traditional use of marketed medicinal juniper (Juniper communis) for mild gastrointestinal disorders such as dyspepsia and bloating in Poland since 1955 and in Germany since 1986 (1).

Clinical evidence for the efficacy of juniper specifically for heartburn is not easily accessible; however, juniper berries contain compounds such as terpenes and flavonoids that have been studied for their potential anti-inflammatory and digestive properties. Proponents describe that these compounds could relieve heartburn symptoms by reducing inflammation and promoting digestion.

The juniper berries (Juniper communis) used to manufacture Roleca contain at least 10 ml/kg of essential oil, the components of which are listed below with the percentages required by the Ph. Eur. standard:

  • α-Pinene (20 to 50%)
  • Sabinene (maximum 20%)
  • β-Pinene (1 to 12%)
  • β-Myrcene (1 to 35%)
  • α-Phellandrene (maximum 1.0%)
  • Limonene (2 to 12%)
  • Terpinen-4-ol (0.5 to 10%)
  • Bornyl acetate (maximum 2.0%)
  • β-Caryophyllene (maximum 7.0%)

Other components such as tannins, flavonoids, diterpene acids, aldehydes/alcohols, and fatty alcohols have also been listed (1, 2).

A study analyzing the essential oil of Pinus koraiensis found that the most effective fraction studied contains many of the same compounds as juniper essential oil, including α-pinene, β-pinene, α-terpineol, and limonene (3). The anti-inflammatory effect of pinenes (one of the most common compounds in juniper oil) is particularly well-documented in both animal (4, 5, 6) and human models (7). The signaling pathways involved in these properties were described in detail in a study on mouse macrophages. It was found that the addition of α-pinene suppresses inflammatory markers (decreased levels of interleukin-6, tumor necrosis factor-α, and nitric oxide, inducible nitric oxide synthase, and cyclooxygenase-2) by acting on mitogen-activated protein kinases (MAPKs) and nuclear factor-kappa B (NF-κB) signaling pathways (8).

Inflammation of the esophagus, often caused by repeated contact with stomach acid due to GERD, can lead to symptoms such as heartburn. Additionally, certain foods and beverages such as spicy foods, citrus fruits, tomatoes, caffeine, and alcohol can trigger inflammation in the digestive tract and contribute to heartburn symptoms (9). Therefore, reducing inflammation through the use of juniper berry oil may help patients relieve their heartburn.

Furthermore, heartburn episodes are often associated with persistent esophageal contractions. Juniper oil has been shown to have spasmolytic properties, as it causes relaxation of potassium-induced contractions in rabbit jejunum preparations at concentrations similar to papaverine (2). This provides a further pharmacological basis for its traditional use in intestinal disorders such as dyspepsia and associated heartburn.
